- Provides therapeutic interventions in the home or community and linking clients to resources
- Assesses clients’ development, psychological and psychiatric needs in order to make referrals.
- Advocates on behalf of clients.
- Monitors clinical symptoms.
- Functions as a gatekeeper for other resources.
- Performs crisis intervention, as needed.
- Meet monthly productivity metrics
Position Requirements:
- High School Diploma or equivalent, Associate’s Degree preferred, with 3 years of related experience
- QMHS/QBHS within 15 days of hire (employer provided)
- Demonstrated passion to advocate on behalf of others
- Demonstrated self-starter and independent thinker who can work in the field as well as in the office
- Organized with good time management skills necessary
- Must have an acceptable driving record 4 (four) points or less, reliable transportation and carry automobile insurance with minimum liability of $100,000/$300,000, and property damage of $50,000.
