Summary:
The Medication History Technician is responsible for interviewing patients to obtain a medication history of patients admitted to Pardee Hospital. The technician will also be responsible to make use of available resources to verify the accuracy of a medication list.


Responsibilities:
• Conducts an appropriate and respectful interview of the patient (or family member if necessary), contacting outside healthcare providers and pharmacies when necessary to clarify/verify patient reported lists, and documents communications in the EMR.
• Accurately records current medications (prescription and OTC), supplements, vitamins, vaccines, and herbal remedies in the EMR. Communicates to the providers or Pharmacists any missing information or details that may be of importance. Refers all clinical questions to a Pharmacist or provider, whichever is appropriate.
• Collaborates with provider staff to identify, clarify and communicate missing or clinically significant patient history information.
• Records or updates medication allergies and any adverse reactions into the EMR.
• Consistently demonstrates good time management by making priorities, working in an efficient and productive manner, and reducing non-essential interruptions to maintain efficient workflow in order to complete medication histories in a timely manner.
• Performs other duties as assigned and readily accepts direction from management, responding in a prompt and positive manner.

Qualifications

Hired candidates will be required to register with the Board of Pharmacy within 30 days of hire for a nominal fee

Required
• Certified Pharmacy Technician (CPhT) from the national Pharmacy Technician Certification Board (PTCB)
• High School Diploma or equivalent.
• Three (3) years of experience working in a customer service capacity.
• Three (3) years of experience working as a pharmacy technician.
• Must have proficient pharmaceutical mathematics skills, including but not limited to multiplication, ratios, and percentages, as assessed by the CPhT test.
Preferred
• Formal training as a pharmacy technician from an educational institution or hospital preferred.
