JOB PURPOSE/SUMMARY

Summary of role of team:

  • Serves as the primary clinician that provides individual, group, and family counseling as part of the First Episode Psychosis/On Track TN model.
  • Clinician will also serve as the team leader and primary supervisor for the OnTrack TN team and facilitate the coordinated specialty care team.
  • Clinician provides weekly individual, group, or family psychotherapy to adolescents and young adults ages 15-30 with a mental health diagnosis that involves symptoms evident of psychosis.
  • Clinician works in a team format to meet the individual needs of clients in the program, while providing clinical recommendations for treatment.
  • Clinician will help clients clarify goals, cope with stressful situations, interact more effectively with other people, and generally to overcome barriers to client's recovery Clinician is to provide psycho-social assessments for clients and provides diagnosis and appropriate referrals for clients assessed, as needed.
  • Clinician provides crisis intervention and emergency services as needed.
  • The primary clinician will rotate in a 24 hour/7 day a week on-call schedule.

Summary of position:

  • Treatment modalities used by Master's level clinicians providing therapy services include Dialectical Behavior Therapy (DBT), Cognitive Behavior Therapy (CBT), CBT for psychosis (CBTp), Trauma-Focused Cognitive Behavior Therapy (TF-CBT), Attachment, Self-Regulation and Competency (ARC).
  • Functions as a member of a treatment team to plan, implement, and evaluate successful interventions for individuals and families.
  • Clinician embraces the key values of clinical services: empowerment, normalization, rehabilitation, and continuity of care.
  • Provides administrative duties for the OnTrack TN program including supervisory responsibilities, training of new staff members and continued staff development, monthly reporting in house and to the state of Tennessee (TDMHSA), and the promotion of the program with outside agencies.
  • Conducts administrative and clinical supervision to a coordinated specialty care team.

  • Work effectively as a team contributor on all assignments.
  • Work independently while understanding the necessity for communicating and coordinating work efforts with other employees and organizations.
  • Conducts individual, family, and group counseling
  • Is responsible for maintaining a designated amount of direct service time with clients and a designated amount of contacts for the assigned caseload.
  • Participates in the initial individual program planning process with all service providers (i.e., Outreach/Education specialist, SEES, CPRS, and medical staff) and provides ongoing information regarding client progress to the service providers.
  • Completes an individual treatment plan with each client within 30 days of enrollment.
  • Writes progress notes after each service contact with a client in accordance with the Center's procedure on Time Frames for Clinical Records and Related Documentation.
  • Maintains documentation of a six-month service update in the clinical record.
  • Participates in the development of a crisis/safety plan for priority clients.
  • Participates in regularly scheduled Peer Review.
  • Follows all guidelines outlined in the HRMC Policies and Procedures Manual including Corporate Compliance Policies.
  • Is expected to have regular and predictable attendance and the ability to work cooperatively with others.
  • If licensed, signing the non-licensed clinicians' treatment plans and being available for clinical consult.
  • Coordinates and schedules ongoing outreach/referral meetings with community providers as part of the OnTrack model.

TYPICAL WORKING CONDITIONS/ENVIRONMENT

  • This position will be housed in the Merchants Campus and requires both office-based clinical and administrative work and community-based clinical interactions.
  • The OnTrack Team Leader will be responsible for engaging clients, peers, and community stakeholders in a professional and therapeutic manner.

COMPENSATION:

  • Starting salary for this position is approximately $52,535 /yr based on relevant experience and education.

Schedule:

  • This position provides services Monday-Friday during regular business hours and also includes an on-call component during which the clinician would be available to respond to after-hours crises by phone or in person as needed.

Travel:

  • This position requires utilizing a personal dependable vehicle to conduct Center business.
  • Maintaining a dependable vehicle and certified driver status is a condition of employment.

Equipment/Technical Competency:

  • Ability to use a computer and basic office software is required, including but not limited to Office Word, Excel, Outlook and be able to utilize EMR for clinical documentation.

Equipment/Technology:

  • This position requires the use of computer skills with an electronic medical record (EMR), and telehealth/televideo services may be offered as part of the service delivery.
  • Staff will be expected to maintain work with a Center cell phone and computer.

QUALIFICATIONS - OnTrack TN Team Leader

Experience / Knowledge:

  • Master's degree in Human Services or related field.
  • One-year experience in the social work field or a behavioral health setting preferred.
  • Excellent verbal, written, and computer skills are necessary in order to work effectively and collaboratively with clients, collaborating agencies, and community professionals, as well as maintaining proper documentation.
  • Clinical staff members must possess reliable transportation, a valid driver's license with an F endorsement OR the ability to acquire an F endorsement on driver's license in order to transport clients as needed.
  • Transporting clients in a personal or Center vehicle is an essential function of this position.

Education / License:

  • Master's Degree in Human Services or related field with course work or experience in the areas of cultural diversity, human development, etiology and treatment of mental illness, alcohol and drug abuse, physical and sexual abuse, suicide, and developmental disabilities.

Physical/Emotional/Social - Skills/Abilities:

  • Knowledge of etiology and treatment of mental illness, physical and sexual abuse, suicide, human development, and cultural diversity.
  • Exposure to biological hazards.
  • Hearing of normal and soft tones.
  • Close eye work.
  • Valid driver's license- F endorsement required for the transportation of clients.
  • Lifting up to 50 lbs.
  • Pushing/pulling up to 150 lbs. frequent sitting, standing, walking, bending, stooping, and reaching.
  • An essential job function is a reliable, predictable 40 hours per week job, on site and in the community, with regular attendance.
  • Ability to effectively and ethically counsel children and families.
  • Ability to present professionally and work within a team format to plan, implement, and evaluate successful interventions.
  • Ability to effectively run and process groups.
  • Ability to work within a team format to meet positive goals for children and adults and interface with other agencies involved in the ecology of the individual.
  • Ability to exercise sound judgment and effective decision-making, ability to set and demonstrate appropriate boundaries, ability to be an empathic listener, flexibility, willingness, and adaptability to working with diverse populations.
  • Must also have the ability to communicate effectively and possess good time management and organizational skills.
  • This position requires training and certification in and the ability to adequately implement a variety of verbal de-escalation methods and personal/client safety techniques.
