Job Summary

The Michigan Alzheimer's Disease Center (MADC) is seeking a part time (24 hours/week) Wellness and Support Program Navigator to join its team. This position is hybrid and will involve remote work as well as occasional work at the Michigan Alzheimer's Disease Center in Ann Arbor where there is free parking, and at various locations in the community. Hours are Monday - Friday, 8 am to 5 pm.

We are seeking an experienced, positive, highly motivated, organized, autonomous person with excellent multi-tasking ability and record-keeping skills to work within the MADC's Outreach, Recruitment, & Engagement (ORE) Core. The Wellness and Support Program Navigator will be responsible for working with the ORE Core team to expand the Center's Wellness and Rinne Lewy Body Dementia Initiatives and related projects, as well as lead programs that the Center offers currently related to participant/caregiver wellness and support. The candidate will facilitate support groups and other programming intended to assist individuals living with cognitive changes and their care partners. The characteristic duties and responsibilities of this position may evolve over time to match changing needs and priorities.

Responsibilities*

  • Work with Center leadership and ORE Core team on creating programming focused on education and support for caregivers, persons living with dementia, older adults, and youth that aligns with the MADC's vision for its growth. 
  • Create and implement Center programming for individuals living with early stage/mild memory loss in the community and collaborate with Wellness Initiative Managing Director to create impactful psychosocial educational programming for care partner dyads. 
  • Lead the planning of wellness education programs related to risk, targeted at aging individuals in the community. 
  • With support from ORE Core Program Manager and MADC Administrative Manager, manage Rinne Lewy Body Dementia Initiative programming, including liaising with the national Lewy Body Dementia Association on various projects related to support and educational programs and LBDA Research Center of Excellence participation. 
  • Create educational and support programming for the Rinne Lewy Body Dementia Initiative and facilitate monthly support groups for persons living with Lewy body dementia and their caregivers statewide. Lead expansion of future Center support group offerings. 
  • Provide ongoing consultation, resources, and connection to appropriate support programs for research and program participants as well as their caregivers and families.
  • Work with ORE Core Program Manager and Wellness Initiative team to enhance the ties between Center wellness programs, support groups, and research. 
  • Register interested participants in caregiver programs offered through the MADC Wellness and Rinne LBD Initiatives and gather/enter data following programs into Center database for analysis. 
  • Participate in and support community outreach activities. 
  • Perform general office and administrative duties, and other duties, as assigned.

Required Qualifications*

  • Master's degree in social work. 
  • Minimum 4 years of experience in a related field.
  • Interest working with older adults with and without dementia and with dementia specific programs, care, support, and outreach.
  • Strong interpersonal, oral, and written communication skills, with experience and interest in public speaking in community-based settings and group facilitation. 
  • Excellent organizational skills, professionalism, attention to detail, ability to work independently but also within a team, personable, and the ability to prioritize multiple tasks and meet deadlines. 
  • Eagerness to learn and ability to adapt to changing needs. Flexible and willing to take on new projects as needed.
  • Ability to work with diverse teams of people in a diplomatic, collaborative and effective manner.
  • Ability to work in a semi-independent manner with minimal supervision.
  • Dedication to community-engaged research practices and cultural humility.
  • Demonstrated understanding of Good Clinical Practice and HIPAA.
  • Computer proficiency (i.e., Microsoft Suite, Dropbox.)

Desired Qualifications*

  • An active clinical license in the State of Michigan (e.g., LMSW, LLMSW).
  • Experience working with older adults with and without dementia. 
  • Experience working with dementia specific programs, care, outreach, and support. 
  • Experience using REDCap.
  • Knowledge of Michigan Medicine policies and procedures.
