The New Jewish Home Certified Home Health Agency (CHHA) provides home based care and rehabilitation therapy for patients recovering from an acute episode or other adverse health event. The New Jewish Home has long experience delivering rehabilitation and home care services. Our goal is to help each patient get well, stay well and return to health and independence as quickly as possible.
The CHHA is currently seeking a part time , 20 hours per week, Physical Therapist to join their team!
Job Summary: Responsible for providing and coordinating multidisciplinary health services for patients in the community. Performs the role of Physical Therapist and works with the physician and other team members in the implementation of plan of care.
Duties and Responsibilities:
- Collaborates with physicians, patients and their families, nurses, therapists, social workers and other members of the patient care team to develop and implement patient specific care and treatment plans.
- Utilizes varied solutions to educate and promote the health and independence of each patient.
- Assess, monitor, document and report progress of patients’ health and condition using required documents, reports and established timelines. Provides supervision and evaluation of aides and physical therapy assistants.
- Observes signs and symptoms of patients, reports to the physician and/or other appropriate staff as often as needed or as change in the patients’ condition requires.
- Completes OASIS assessments timely, accurately and according to OASIS user’s manual definitions. Completes all documentation, patient health status reports as required and forwards appropriately.
- Supervises occupational therapy assistant according to state regulations and agency policy.
- Assists with the coordination of the total plan of care and maintains continuity of patient care by liaising with other appropriate staff assigned to the same patients. Participates in team/case conferences. Initiates team/care conferences for complex and/or multi
- disciplinary patients when required to ensure coordination of care.
- Performs other duties as assigned.
Specialized Skills:
- . Demonstrates ability to perform skilled tasks of a Physical Therapist , including, but not limited to:
- a) Ability to complete comprehensive patient evaluation.
- b) Ability to develop and implement individualized treatment plan for a variety of patient presentations.
- c) Ability to modify treatment plans and goals based on patient response.
- d) Ability to establish measurable goals with achievement dates.
- 3. Demonstrated ability to direct, guide and instruct others.
Requirements:
- Must possess a current, unencumbered license to practice as Physical Therapist in the State of New York.
- Successful completion of an occupational therapy program at a school of occupational therapy approved by the American Occupational Therapy Association or the Council on Medical Education and Hospitals of American Medical Association or the committee on Allied Health Education and Accreditation of the American Medical Association.
- Master’s degree or Doctorate degree in Occupational Therapy.
- One year of occupational therapy experience preferred.
