Summary:
The CAP/DA Case Manager provides formal assessment, determines Medicaid or other payer source eligibility, develops care plans, financial budget and provides case management services for individuals in need of supportive services to avoid institutionalization or allow discharge from an institution.
Responsibilities:
Patients/Families are served in providing assistance by case management. Performs patient interviews and conducts interpersonal relationships in a manner to project a positive image of the department and hospital. Ensures that contacts with physicians, community agencies and other healthcare team members are friendly, helpful and courteous.

EDUCATION REQUIREMENTS
- Certified Social Worker or BSW. RN must meet NC Office of State personnel job specifications for a Public Health Nurse I or II or higher.
PROFESSIONAL EXPERIENCE REQUIREMENT
- Three years in a Social Work setting or three years in Home Service Agency. Case Management in an acute/long term care setting.
LICENSURE/REGISTRATION/CERTIFICATION
- Certification in Social Work. Current license to practice as an RN in the State of North Carolina.
KNOWLEDGE, SKILLS AND ABILITIES REQUIREMENTS
- Good physical, mental and social assessment skills. Understanding of patient care in the home environment according to developmental stages for adult and geriatric population. Reading, Mathematics, Communication skills. Must be able to work autonomously.
WORK ENVIRONMENT
- Exposure to airborne and bloodborne pathogens, exposure to electrical hazards (electrical use).
PHYSICAL REQUIREMENT
- Standing, Walking, Sitting, Reaching, Talking, Seeing, Hearing, Tasting, Smelling, Carrying, Pushing, Pulling, Lifting, And Moving.
